The intent of the Licensed Provider Search is to permit the public to view the inspection and investigation reports conducted by the Office of Licensing and the Office of Human Rights, as warranted, for providers licensed by the Department of Behavioral Health and Developmental Services. Only investigations and unannounced inspections with violations will have a report.
